Healthcare Navigation Systems Medical Clinic (“the Clinic”) provides membership-based access to primary care and wellness-related services. Services are designed to support general health, wellness, and preventative care and do not replace emergency medical services.
Membership services are not health insurance, are not a substitute for insurance, and do not meet any federal or state insurance mandate requirements.
Membership fees cover access to services only and do not constitute insurance, prepaid health coverage, or a health plan.
The Clinic does not bill insurance on behalf of members for services included in the membership unless otherwise stated in writing. Any services not included in the membership may incur additional fees.

Wellness consultations
Preventive health guidance
General health assessments
Services included vary by plan and are subject to availability, provider discretion, and clinical appropriateness.
The Clinic does not guarantee specific outcomes, diagnoses, treatments, or results.

Information provided through consultations, materials, or communications:

Members are encouraged to seek immediate emergency care by calling 911 or visiting the nearest emergency facility for urgent or life-threatening conditions.
Membership fees are billed on a monthly recurring basis
Fees are due regardless of service utilization
Prices are subject to change with prior notice
Failure to maintain payment may result in suspension or termination of membership access.
Memberships may be canceled by the member with written or electronic notice in accordance with the Clinic’s cancellation policy.
Membership fees are non-refundable once a billing cycle has begun
Access remains active through the end of the current billing period
No refunds are issued for unused services
Services not included in a membership plan—such as specialized testing, procedures, medications, referrals, or external services—may incur additional charges.
Members are responsible for any fees associated with services outside the scope of their plan.

The Clinic reserves the right to refer members to external providers when necessary.

To the fullest extent permitted by law, the Clinic is not liable for:
Indirect or incidental damages
Outcomes beyond the Clinic’s reasonable control
Services rendered outside the scope of the membership
Membership participation is voluntary and subject to these Terms & Conditions.
The Clinic reserves the right to modify these Terms & Conditions at any time. Updates will be posted on the website and become effective upon publication.
Continued use of services constitutes acceptance of the revised terms.
These Terms & Conditions are governed by the laws of the state in which the Clinic operates, without regard to conflict-of-law principles.
